Case Management Services
SJCS-TN Case Management assists persons a) 65 years and older or b) 21 years and older with a physical disability to find personal, health and household support to enable them to remain in their home environment instead of a nursing home. The services, carried out by qualified SJCS-TN Case Managers, are designed to ensure that each person receives the support based on his or her individual care plan. Eligibility is determined by the Aging Commission Area Office and funded by the State of Tennessee.
Referrals are made to authorized provider agencies for an array of services, such as in-home respite care, homemaker, personal care, home-delivered meals, and adult day care. Our goal is that all services received will support a person to live happily and safely in his or her home environment.
